Below is the minimum requirement of diagnostic testing that must be received/completed prior to the initiation of Testosterone Replacement Therapy at The Body Mod. Any missing lab results will be ordered and must be completed before initiation of treatment can occur. 

  • CBC with differential
  • Complete Metabolic Panel
  • Lipid Panel
  • LH/FSH
  • Prolactin
  • SHBG (Sex Hormone Binding Globulin)
  • Total Testosterone
  • Free Testosterone
  • Estradiol
  • PSA (total and % free)

To provide recent lab results completed within the last 3 months, you may:

  1. Upload any recent labs into your Patient Portal
  2. Have your PCP fax them to 937-530-0070.
  3. Provide a share code to our provider to allow 1-time access to download your recent lab work. Please contact the office for instructions to ensure a provider is available, share codes are only valid for 1 hour.

Additional labs will provide a better evaluation of your health status and may be requested during your Initial Consultation.  

Lab monitoring is completed 6 weeks after initiation of treatment and 3 months after any changes in treatment. You’ll also complete a questionnaire to monitor symptoms/resolution. Once stable, diagnostic testing will be completed every 3-6 months or as needed.

*You may sign a waiver for the following results, but you agree you will provide a copy of the most recent results at your next follow-up appointment.

To share recent lab results completed within the last 3 months, you may:

  1. Upload any recent labs into your Patient Portal

Your prescription is submitted to the compounding pharmacy once your subscription has been initiated. After the pharmacy receives the order, average turnaround is 5-7 business days due to the personalization of your medication.

Be aware that most pharmacies are not open on weekends and/or holidays and you should anticipate longer delivery times during holidays. 

Refills are ordered the day of your follow-up visit. Shipping charges for specialty medication will be applied for $12 for 2-day shipping.

Not necessarily. The amount of medication you receive is based on your subscription length and your personalized dosing schedule.  Injectable medication is dispensed in increments of 1mL.

Based on your current dose, you may receive more medication than necessary to keep shipping costs down,  which will be accounted for during your next month’s dosing schedule.

Testosterone is dispensed in increments of 4mL and 5mL vials, which lasts approximately 6-8 weeks. For this reason, you receive more medication than necessary for your subscription length. This amount of testosterone is accounted for during your next month’s dosing schedule as Testosterone is a schedule III medication and monitored through the state’s automated prescription reporting system for controlled substances.

For quarterly subscriptions, 10mL at a time are sent (which typically lasts 14-16 weeks) in an effort to keep shipping costs down and to avoid a lapse in treatment due to holidays/weekends/mail problems, etc. 

The monthly or quarterly subscription fee is in exchange for The Body Mod providing and refilling prescription medications, hormone replacement therapy, routine visits, and electronic communication with the Provider and staff to manage your treatment regimen.

Clients are contractually responsible for the monthly or quarterly subscription fees regardless of when prescriptions are issued or follow-up visits are performed.

A typical progression of symptom relief with TRT may look like this: 

  • In 4-6 weeks: Psychological changes. Improved focus and concentration, mood is generally uplifted.
  • In 6-8 weeks: Physical changes. Decreased weight, increased energy, sexual performance changes, starting to build muscle mass.
  • In 8 weeks-6 months: Overall consistent improvement in daily functioning with an increased sense of well-being that will persist for the duration of therapy.

Unfortunately, this is a complex question which is determined by the individual’s medical history, co-morbidities, status of nutritional deficiencies, stress management, dietary choices, physical activity level, etc. It is our goal to get you feeling better and have symptom resolution as quickly and as safely as possible.

The Body Mod has a 24-hour cancellation policy.  

When you schedule an appointment with us, this appointment time is set aside especially for you. It is not available to other patients. Therefore, it is important that you let your practitioner know as soon as possible if you will be unable to come to your scheduled appointment. This allows us to make this time available to other patients. As a courtesy to the providers at The Body Mod and other patients, please provide adequate notice (24 business hours for follow-up appointments and preferably 48 business hours for new patient appointments) if you need to cancel or change an appointment.

  • Cancellation/Rescheduling Policy: You may cancel or reschedule your appointment in a timely manner via the Patient Portal or by calling the office. Out of courtesy to other patients and our providers, we ask that you please notify the office as soon as possible if you need to cancel or reschedule your appointment.
      • For Existing Patients:
          • If you do not provide adequate notice, you will need to call the office and you will be charged a $50 late-cancellation fee to the authorized card on file.
          • Voicemail and text messages left after hours the day before your scheduled appointment will be considered a late-cancellation and will result in a late-cancellation fee.
      • For New Patients:
          • Failure to complete required consent, medical history, and demographic information forms 24 hours prior to an Initial Consultation will result in automatic cancellation of the Initial Consultation and a $50 late-cancellation fee will be charged to the card on file. In the event there is no card on file, $50 will be deducted from the previously paid consultation fee. As a result, you will need to contact the office at 937-530-0070 to complete an additional $50 payment for your Initial Consultation fees in order to reschedule your appointment.
          • A second offense of late-cancellation can result in dismissal from the practice.
  • True emergencies are the exception to this policy. In the event of a true emergency, this policy will be waived. Please note: work and school-related demands or obligations are not considered true emergencies.
  • While we do provide appointment reminder notifications, please be aware that this is provided as a courtesy to you. It is not our responsibility nor an excuse for missing an appointment.

We do not want late-cancellations or missed appointment fees to be an impediment to your care and so we ask for your mutual respect. When you make an appointment, we block out our schedule for you and ask that you exercise the same consideration when planning your calendar. Thank you for your respect and understanding.

If you know you are going to be late, please let us know as soon as possible, and we will try our best to accommodate before rescheduling and assessing the late-cancellation fee.

  • Late Arrival Policy:
      • For Existing Patients
            • Arriving 10+ minutes late may result in a cancelled appointment for which we will charge a $50 late-cancellation fee.
            • Patients who habitually arrive late can be dismissed from the practice after the second offense.
            • As a courtesy to you, we understand that your time is valuable as well, so we will do our very best to stay on schedule.
      • For New Patients:
            • Arriving less than 30 minutes late to your Initial Consultation will result in a condensed appointment which may require scheduling a follow-up appointment for a thorough review, depending on complexity of concerns.
            • Arriving 30+ minutes late to your Initial Consultation will result in automatic cancellation of the Initial Consultation and a $50 late-cancellation fee will be deducted from the previously paid consultation fee. As a result, you will need to contact the office to complete an additional $50 payment for your Initial Consultation fees in order to reschedule your appointment.
  • True emergencies are the exception to this policy. In the event of a true emergency, this policy will be waived. Please note: work and school-related demands or obligations are not considered true emergencies.
  • While we do provide appointment reminder notifications, please be aware that this is provided as a courtesy to you. It is not our responsibility nor an excuse for arriving late to an appointment.

  • No-Call, No-Show Policy:
        • For Existing Patients:
              • If you fail to show up for an appointment without notification, you will be charged a $50 no-show fee to the authorized card on file and written acknowledgement of concern with a copy of the policy.
              • A second offense of no-call, no-show can result in dismissal from the practice.
        • For New Patients:
              • If you fail to show for an appointment without notice, you will forfeit your entire prepaid consultation fee. As a result, you will need to contact the office at 937-530-0070 to complete an additional $199 payment for your Initial Consultation fees in order to reschedule your appointment.
  • True emergencies are the exception to this policy. In the event of a true emergency, this policy will be waived. Please note: work and school-related demands or obligations are not considered true emergencies.

There is a 3 month minimum commitment with any subscription service. You may cancel the subscription at any time afterwards, but we do require a 30-day notice in writing. You may continue to use the services of your membership after submitting the request to cancel until the last day of your paid subscription. 


Please note: After giving a 30-day written notice of cancellation, you are still responsible for payment until that time. If you are due, a refill will be sent the day of your follow-up visit. No further refills on specialty medications will be sent after the cancellation notice has been received. 

Please be aware that due to the nature of our services and the personalization of your prescriptions, in the event that you cannot tolerate or do not wish to take the medication customized for you, we are not able to accept any returns or offer any refunds, regardless of satisfaction or results achieved.


If you need us to dispose of the medication properly, please feel free to drop it off at the office during business hours.
